Riding the longest TdF stage with pneumothorax incurred in a crash at the start.  In layman's terms, that's 147 miles with a collapsed lung.

Reto Hollenstein, IAM touched wheels and crashed at the start today.  Was delayed 5 minutes.  Chased for 60k to rejoin the peloton and finished with the grupetto, finishing 27 minutes down in 161st place.  6 hours, 47 minutes of racing with a collapsed lung!

I wish I could find the photo of that soccer player feigning injury!
