Next year my wife and I plan on a trip to Italy and SHE suggested we plan it around Milan-San Remo so I could ride part of the course, watch the race and then head off to see other parts of Italy.

I know some of you have done events similar to that.

I'd like to do one of the pre-race rides on a portion of the course. Where do I start? Recommendation on where to watch live?

For getting most out of watching live I'd recommend a spot were decisive action is to be seen. Probably wither steepest part of Poggio or else the finishing straight. Ideally combined with a spot that has a life screen and some beverages. Those ingredients were easily combined on Kwaremont but getting that for a linear race like MSR will be harder..

For riding I'd start where the course hits the hills and coast. The rest is boredom, relatively speaking. Pretty sure there's a cyclo event but would have to look.

There is indeed a granfondo, http://milano-sanremo.org/en/

However it is in June, and covers the entire parcours, making it the longest GF in the world. As OC pointed out, the majority of the course is not all that interesting. Participants also need to either reserve a place on the bus back to Milan, or have a friend or family member drive to the finish and meet them there.

As you know also from watching past editions, weather in March can be unpredictable.

There's also a Gran Fondo around San Remo day after the race, 120k takes in most of finale and a bit of spectacular Maritime Alps, +2400 m:

You could come in through Nice, which is not far from San Remo.
